Pre-Registration/ Online Scheduling Instructions
You have the option to pre-register for any test to expedite your entrance into the hospital (both inpatient and outpatient) and to schedule your mammogram online.
If you would rather, you can also download the pre-registration form and bring it with you.
Our Admissions Department is open:
- Mon - Fri: 7:00am - 5:00pm (main lobby)
- Weekdays after 5:00 pm: Register in the Emergency Department
- Saturday: Register in the Emergency Department
- Sunday: Register in the Emergency Department
Either way you choose to register, please remember to have these items
on hand:
- Valid picture ID
- Copy of your insurance card
- Co-payment/deposit
- Consent signature
- Physician's order
Insurance Information
We accept most HMO/PPO insurances, most types of Medicaid, and all Medicare patients. If you have questions about your coverage here at EIRMC, please call your insurance company. The phone number should be listed on your insurance card.
Prior Authorization
Authorization is a requirement of many insurance companies, and will vary based on the procedure. Your physician’s office can assist you with pre-authorization for ELECTIVE procedures.
Deposits/Co-Payments
Deposits/co-payments are determined by your insurance company and based on specific plan benefits. For most services, the co-payment will be listed on your insurance card. Co-payments/patient portions are due at the time of service.
Self-Pay patients: Patients without insurance are required to make a deposit for treatment.
Outpatient Procedures
Outpatient procedures are generally diagnostic in nature and do not require an overnight stay.
Refer to the information above to pre-register for your scheduled outpatient procedure. If you need Laboratory, Labor & Delivery or Radiology services during closed hours of business, please go directly to those departments. The Admissions Department will register you by telephone.
Maternity
Congratulations on your impending arrival! Completing a pre-registration at the beginning of your second trimester or at the time you select EIRMC for your delivery helps make the big day easier for you. You can focus on your birthing experience instead of the paperwork (we’ll handle that part for you!).
You can pre-register online in the convenience of your own home by clicking on the link above. If you prefer, you can instead get the pre-registration packet at EIRMC’s Main Registration department or through your OB/GYN office. These forms gather basic demographic & insurance information for registration completion and insurance verification.
Once completed, mail them back to the address below or drop them off at our Registration office in the main lobby.

Since our first priority is your health and comfort, please go directly to the Women & Infants Center when you are in labor. If a family member or spouse is with you, we request that they visit Registration. This allows us to verify demographic information and complete the required consents.
Surgery
Pre-registering before your surgery allows you to discuss issues of concern with registration/admission employees prior to arrival for their procedure, talk directly with a financial counselor, and learn about discounts available for needy or uninsured patients.
If your surgery is already scheduled, click on the link above to pre-register online. You will receive a follow-up call from our Admissions Department to talk through any additional information you will need.
If you prefer, you can instead call 529-6240 to pre-register, or you may visit the Admitting office in the hospital lobby for a face-to-face pre-registration. Any necessary pre-operative tests will be scheduled at this time.
Regardless of how you pre-register, on the day before your surgery, a member of the Day Surgery staff will call to inform you of your arrival time at EIRMC.
On the day of your surgery, please go directly to Surgical Admitting. An Admissions Representative will come to your room before the surgery to get copies of insurance cards, signatures, and collect co-pays.
You will be scheduled for surgery with information obtained from your physician’s office.
