For our patients who have been admitted with COPD, once you are discharged the excellent care doesn’t stop there. EIRMC’s COPD clinic is a post-hospital discharge follow up service that ensures support for our COPD patients.

Using the standards from the Global Initiative for chronic Obstructive Lung Disease (GOLD) our COPD clinic has been carefully designed to meet the needs of COPD patients and incorporate several resources in the outpatient setting to ensure your success. Our COPD clinic team is made up of physicians, nurse practitioners, respiratory therapists, nursing, exercise physiologists, dietitians, and more.

Upon discharge, your enrollment in the program begins with a comprehensive assessment by the clinical team. We will help make sure that you have everything in place to be successful at home. The clinics program and resources are tailored to the individual needs of each patient and can range from weekly in clinic visits to telephone follow ups.

The COPD clinic also coordinates with our communities various home oxygen companies to help ensure that all of your home oxygen equipment is setup and running smoothly. Working directly with these various companies allows us to ensure that you receive the best care possible.

Perhaps the most noteworthy aspect of the COPD clinic is the support it receives from our high quality respiratory therapy department. Our respiratory therapists will assist you during your inpatient stay, help prepare you for discharge and enroll you in the COPD clinic by working directly with the clinic staff.

  2. Make sure you’re set up with the right equipment at home
  3. Detect and address when your COPD worsens of “flares-up”
  4. Provide education
