My Medical Records

EIRMC values your privacy rights and takes many precautions to safeguard your health information.  We want the right people to get information necessary for your care, and make your access to information as convenient as possible.  

Your medical record contains documentation by all physicians and healthcare providers involved in your care and treatment while you were here at EIRMC.  It contains the results of diagnostic tests and therapeutic procedures performed, as well as information about your medical history, and your condition and progress during your hospitalization.  

How Do I Receive My Medical Record Information?

Your medical record, and the information it contains, is confidential.  Therefore, we must confirm your identity before releasing your medical record.  You can download the authorization form here.   Only the patient or his/her legal guardian can sign the form. 

To obtain your medical record, bring the completed and signed authorization for and a picture ID to the Medical Records Office at EIRMC.  Copies of test results or other documents from recent hospital stays can usually be provided at this same time. There will be a charge of $0.25 per page for requests of entire medical records or requests.

If your request is for records more than a year old, or for multiple and extensive stays, it may take up to 30 days to reproduce the records. 

Office Hours & Location

The Medical Records Department is located in the main hospital, near the front lobby.  The front desk volunteers can direct you to the Medical Records office.

Our hours of operation are:

Monday-Thursday, 8:00 a.m. to 5:00 p.m.
Friday, 8:00 a.m. to 4:30 p.m.
